I understand that I or my next of kin will be responsible for obtaining all needed permits, as well as transportation, care for, and sheltering of the body (with ice, or in a cooler at 40° F), or for arranging with a funeral home to provide the same.

I understand that the Cemetery must receive a valid Burial/Transfer Permit on the day of interment, and declare that I will either file all needed documents to obtain this or will arrange with a funeral director licensed in my state to obtain the permit on my behalf. I understand that burial will not be able to proceed without this legally required permit.
